Combined appliances with an extraoral support.

The appliance contains a palatal basis in the form of sagittally mountable rectangular frame with sliding details, which are connected to the fastening elements for teeth through the springs. Fastening elements may be realized in the form of dental plates (mouthguards) (see Fig1) or support-retaining wireframes mountable on the frontal and lateral segments of dentitions. (see, Fig2.)

Fastening elements of lateral segments are also realized with fixed and removable parts (see, Fig4). In this case the arch fixable on the fixed parts of fastening elements is connected to the springs through the palato-alveolar or alveolar plates. (see, Fig3.) Fastening elements of separate segments and their sectors are provided with the units for application of the extraoral traction. The extraoral part of the appliance constitutes a known headgear with a chin cup. (see, Fig5.) If it‘s necessary, such an embodiment of the appliance allows to form sectoral elements of the groups or separate  teeth and by application of intra- and extraoral efforts to achieve their  simultaneous, segment by segment movement in different directions. The construction   of the appliance Mailyan12-3 provides the conditions for increasing the transversal vector of extraoral efforts, which contribute to the intensification of the   median palatine suture rupture. The latter is carried out by simultaneous intra- and extraoral force influence on the median palatine suture through the palatal plates. The construction also allows to achieve the correction of the displaced center of the dentition by the activation of the springs, respectively by applying extraoral efforts on the lateral and the central groups of teeth.
